Mesothelioma Types and Cell Types

Doctors describe mesothelioma in two important ways: where the cancer begins and how the tumor cells look under a microscope. Both classifications can affect evaluation, prognosis, treatment discussions and clinical-trial eligibility.

Types by location

Pleural mesothelioma

Pleural mesothelioma develops in the pleura, the tissue lining the chest cavity and surrounding the lungs. It is the most common anatomical form. Possible signs include shortness of breath, chest pain, cough and pleural effusion, which is fluid around a lung.

Pleural mesothelioma is commonly staged using the TNM system. Treatment may involve systemic therapy, surgery in selected patients, radiation, symptom-directed procedures or clinical trials.

Peritoneal mesothelioma

Peritoneal mesothelioma begins in the tissue lining the abdominal cavity. Possible symptoms include abdominal swelling or pain, nausea, appetite changes, bowel changes, fatigue and unexplained weight loss.

Selected patients may be evaluated for cytoreductive surgery combined with heated intraperitoneal chemotherapy, often called HIPEC. Suitability requires specialist assessment.

Pericardial mesothelioma

Pericardial mesothelioma affects the sac surrounding the heart. It is extremely rare. Because cardiac and breathing symptoms have many more common causes, diagnosis requires specialist evaluation and tissue analysis when feasible.

Mesothelioma of the tunica vaginalis

This rare form affects tissue surrounding a testicle. It may present as swelling or a mass, but those findings are more commonly caused by other conditions.

Histologic cell types

Epithelioid

Epithelioid tumors contain cells that may form recognizable patterns or structures. This is the most common histologic group and generally has a more favorable prognosis than sarcomatoid or biphasic disease, although individual outcomes vary widely.

Sarcomatoid

Sarcomatoid cells are spindle-shaped and may resemble other tumors. Diagnosis can be challenging, especially with small tissue samples. This histology is generally associated with a less favorable prognosis.

Biphasic

Biphasic mesothelioma contains both epithelioid and sarcomatoid components. The proportion and distribution of each pattern may affect interpretation, and adequate tissue sampling is important.

Why expert pathology matters

Mesothelioma can resemble lung adenocarcinoma, metastatic cancer and certain noncancerous processes. Pathologists use tissue appearance, immunohistochemistry and clinical findings together. A second pathology review may be valuable when tissue is limited, results are uncertain or major treatment decisions depend on the classification.

  • Where did the cancer begin?
  • What cell type did the biopsy show?
  • Was there enough tissue for a confident diagnosis?
  • Which laboratory markers support the conclusion?
  • Has a pathologist experienced with mesothelioma reviewed the sample?
  • How do location and cell type affect treatment options?
